Position Summary
In collaboration with the Central South Stroke Network, the District Stroke Coordinator will provide strategic and operational leadership to support an integrated stroke system in Brant Haldimand Norfolk. The incumbent will be reporting to the Director of the Stroke Program and will ensure the provision of best practice stroke care across the continuum of the healthcare system (promotion, prevention, hyperacute, acute, rehabilitation and community programs).
Primary Responsibilities

Develop and sustain formal partnership throughout the district related to stroke care.

Oversee and manage the development of the stroke strategy in consultation with appropriate internal and external key stakeholders.

Assume responsibility and accountability for the overall administration of the project including the professional, technical, staff and financial aspects.

Maintain, monitor, and interpret program reports and statistical data.

Develop work plans and progress reports.

Be responsible for dissemination of information to all participants.

Facilitate implementation of continuing education activities as required.

Responsible to develop and maintain annual budget.

Develop business cases and impact analysis for new projects, programs, and resource allocation.
Mandatory Qualifications
- Bachelor of Health Science, Nursing, or Allied Health is required.
- Regulated Health Professional and Member in good standing with their respective College is required.
- Minimum of 2 years of stroke experience required
- Progressive leadership experience in the healthcare system preferred
- Experience in health service research, program evaluation or project management.
- Demonstrated ability to work both independently and as a member of a team.
- Demonstrated critical thinking, communication, organization, problem solving, teaching and leadership skills.
- Knowledge of data collection and information systems with strong computer and information technology skills.
- Understanding of existing processes/mechanisms related to the continuum of stroke care as described by the co-ordinated stroke strategy.
- Supportive of principles of patient-focused care.
- Excellent attendance record.
